ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

Event Name: Pedal for Purpose fundraiser (the "Event")

Beneficiary: Kidthink Children's Mental Health Centre Inc. ("KIDTHINK")

1. Independent 3rd-Party Event Clarification

By signing below, I acknowledge and agree that Pedal for Purpose is an independent, third-party community fundraiser organized solely by Kevin Molina (the "Organizer").

I explicitly understand that Kidthink Children's Mental Health Centre Inc. (KIDTHINK) is strictly the beneficiary of the funds raised and is not the organizer, host, manager, or supervisor of this Event. KIDTHINK assumes no responsibility, liability, or oversight for the planning, safety, or execution of the Event.

2. Assumption of Risk

I understand that participating in a cycling event carries inherent risks, including but not limited to:

• Dehydration, heat exhaustion, or unpredictable weather conditions.

• Equipment failure or mechanical malfunctions.

• Collisions with pedestrians, other cyclists, vehicles, or stationary objects.

• Inherent hazards of the road or trail surface (potholes, gravel, debris).

I certify that I am physically fit, have sufficiently trained for participation in this Event, and have not been advised otherwise by a qualified medical professional. I voluntarily assume all risks associated with my participation.

3. Helmet and Safety Requirement

• Mandatory Helmets: I agree to wear an approved, properly fitted cycling helmet at all times while riding during the Event.

• Traffic Laws: I agree to obey all local traffic laws, signals, and instructions given by Event volunteers or coordinators.

4. Release and Waiver of Liability

In consideration of being permitted to participate in this Event, I, for myself, my executors, administrators, heirs, next of kin, and successors, hereby take action to:

• Waive, release, and discharge Kevin Molina, volunteers, event sponsors, and Kidthink Children's Mental Health Centre Inc., including their respective directors, officers, employees, and agents, from any and all liability for my death, disability, personal injury, property damage, or property theft which may occur to me during or traveling to and from this Event.

• Indemnify and hold harmless the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this Event, whether caused by the negligence of releases or otherwise.

5. Medical Treatment Consent

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this Event. I understand that I am solely responsible for any costs associated with such medical care.

6. Media and Photo Release

I understand that during the Event I may be photographed or filmed. I agree to allow my photo, video, or film likeness to be used for any legitimate promotional purpose by Kevin Molina and KIDTHINK, including social media, websites, and future fundraising materials, without compensation to me.
